LWBC Summer Staff Forms

NEW FORM PROCESS: FORMS NEED TO BE IN TWO WEEKS PRIOR TO SERVING
With the hopes of making our form process easier, we have revised it to include only two packets of forms. The first packet of forms is for people that are serving for the first time at Living Waters, this packet includes the full set of forms and will only have to complete the first time you serve at Living Waters. The second packet is for people who have served during past summers and have completed the full packet of forms. This packet will have to be completed yearly and sent in two weeks prior to camp.

Weekly Summer Staff Forms

These forms are for volunteer weekly summer staff.
Staff Manual for all weekly staff:
Staff who've served at LWBC:
(Need one reference, background check, policy card, and health form.)
 
Staff who haven't served at LWBC:
(Need 2 references, application, background check, policy card and health form.)
  


Full-Time Summer Staff Forms

These forms are for staff considering serving the entire summer.
